Facility Evaluation Report
At approximately 9:55 AM, Licensing Program Analyst (LPA) Elias Magdaleno arrived unannounced to conduct a required 1-year annual inspection and was greeted by staff. Director Elfleda Delacruz arrived shortly after. Facility is an Adult Day Program (ADP) with eighteen (18) clients in care. Facility is serviced by vendor North Bay Regional Center (NBRC). Facility operates Monday through Friday 8:00 AM – 2:00 PM.
At approximately 10:10 AM, LPA initiated a tour of the facility with Director and observed the following: Facility consists of one (1) story, was a comfortable temperature, and passageways were free from obstructions. Facility's fire extinguishers were observed charged and last serviced 7/25. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperatures in participant accessible sinks read between 107.2- and 108.1 degrees F which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. Soap & paper towels were available in facility bathrooms. Closets containing cleaning supplies and other items that could pose a risk are kept locked. Participants bring their own lunches and eat in the kitchen. Refrigerators for participants’ lunches were clean and in good repair. Transportation is arranged by a third-party transportation company or participants private transport.
LPA observed a weekly schedule of activities, events, and learning opportunities. These include arts and crafts, movies, music, board games, group activities, and facility maintains multiple computers for participant usage. LPA observed staff conduct group bingo with participants, those who prefer solitary activities were allowed to continue. Staff are assigned groups of participants to provide care and supervision to.
