Facility Evaluation Report
Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct an annual inspection and was greeted by staff Toalima King. Administrator (admin) Dana Thomas arrived shortly after. Facility is an Adult Residential approved for 4 ambulatory clients. Facility is a 4 bed, one story and two bath with 4 clients in care. 2 clients were attending day program while 2 clients were in facility during inspection.
LPA toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed inside facility. All required postings were in a highly visible area. Clients have access to outdoor use with outdoor exits and walkways unobstructed. All bedrooms found in good repair. All bedrooms were observed to have clean linens, night stand, chair and light.
LPA observed at least a 2- day supply of perishable and 7-day supply of non-perishable food. Facility kitchen, refrigerators and freezers were clean. Snacks accessible to clients. Expiration dates were not noted in food (Technical violation given). Toxins are stored in a locked closet and inaccessible to clients. Knife was observed to not be locked in kitchen drawer, admin removed knife immediately (Technical violation given). Emergency food and water observed.
Bathroom accessible to clients did not have paper towels or toilet paper (Technical advisory given) . Water temperature measured within regulation at faucets accessible to clients. Fire extinguishers were charged and inspected on 11/15/2024. Carbon and fire alarms were tested and operational. Disaster Drills are recorded and conducted.
