Facility Evaluation Report

Licensing Program Analyst (LPA) Contreras arrived to conduct a pre-licensing inspection for change of ownership for new facility, Pacific Gardens at Tree Duck (#486804298). LPA met with Residence Care Coordinator and responsibility designee Mary Grace Defeo. LPA Contreras observed the following:

-No bath mats for both bathrooms accessible to clients

-Thickener not stored in locked medication cabinet

-No expiration dates on food items located in refrigerator

- Pre-poured medication in unlocked drawer in kitchen (See LIC809D)

LPA discussed the following items with new administrator and the importance to keep new facility in compliance.

The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties.

Exit interview conducted and appeal of rights provided.
