Facility Evaluation Report

Licensing Program Analyst (LPA) Contreras arrived announced at facility and met with administrator (admin) Lovely Joseph. The purpose of the visit is to complete a final walk through for licensee initiated facility closure.

LPA did a walk through with admin of all the building and grounds. Licensee has not had residents in the last 2 years. LPA found no evidence that would suggest that any residents are residing on the premises.

Licensee turned over license during today’s inspection. The license is now closed and no longer valid, effective today 10/03/25. LPA will send out forfeiture letter to Licensee to complete closure process.
