The inspector’s account

At approximately 11:20PM, Licensing Program Analysts (LPAs) Felias and Contreras arrived unannounced to conduct a Case Management - Incident Visit and met with Administrator Janne Angeles. The purpose of the visit was to follow up on an incident report that was self-submitted to Community Care Licensing (CCL).

Incident Report 1 and 2: CCL received two incident reports on 4/11/2025. On 4/08/2025, Administrator became aware of three incidents that involved Staff Member 1 (S1). Client 1 (C1) informed Administrator of two separate incidents. On one incident, S1 tapped or smacked their buttocks, and the second incident involved S1 rubbed lotion on their stomach due to stomach pain. Client 2 (C2) also informed Administrator of an incident involving S1 where S1 placed their hand inside their clothes by their hip and tapped their buttocks. Reports stated that S1 was suspended pending an internal investigation.

During visit, LPAs were informed that that facility's internal investigation resulted in S1's termination. LPAs requested documents and conducted interviews.

Exit interview conducted. Copy of report and LIC811 (Confidential Names) discussed and provided to Administrator. Signature on form confirms receipt of documents.
