Facility Evaluation Report
Licensing Program Analyst Contreras arrived unannounced to conduct a Case Management- Incident visit and was greeted by staff Jocelyn Bagani, Administrator (admin) Leslie Pascua was unable to attend visit but was available via phone.
On 5/20/2026 CCL received an Incident Report (IR) regarding admin observed behavior and safety concerns involving Client 1 (C1) inviting strangers into facility. Admin has since issued eviction notice. Eviction notice has not been sent to CCL and LPA had conversation ensuring eviction notice is sent to CCL to ensure Title 22 Regulations are followed.
Since incident, admin and staff have continued to monitor facility rules and expectations. Staff claimed no recent strangers have since came to facility.
In addition, LPA interviewed staff in regards to an open investigation regarding facility that was initiated on 5/14/2026.
No deficiencies cited during today's meeting. Copy of report given and read with staff.
