Facility Evaluation Report

At approximately 9:40, Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct an annual inspection and was greeted by staff Jocelyn Bacani, administrator (admin) Leslie Ann Pascua arrived shortly after. Facility is an Adult Residential approved for 5 ambulatory clients. Facility has 5 rooms total with 4 client rooms and 1 staff room in use.

LPA toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed inside facility. All required postings were in a highly visible area. Clients have access to outdoor use with outdoor exits and walkways unobstructed. All bedrooms found in good repair. All bedrooms were observed to have clean linens, night stand, chair and light. LPA observed at least a 2-day supply of perishable and 7-day supply of non-perishable food. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Toxins are stored in a locked closet and inaccessible to clients. Sharps and knives were locked in kitchen drawers. All bathrooms have required bath mat and paper towels. Water temperature measured within regulation at 110.3 degree F at faucets accessible to clients. Fire extinguishers were charged and inspected on 7/15/2025. Carbon and fire alarms were tested and operational. Disaster Drills are recorded and conducted monthly.

LPA reviewed 4 of 4 client records. All documentation was present and physicians reports up to date.

LPA reviewed 4 Staff records, all documentation was present. One staff lacked DSP training due to being out on leave and admin not sure of staff's return. Second staff lacked DSP training due to being newly hired. Admin to schedule training for both staff. Staff had Health Screening from previous employer. LPA had discussion with admin that Health Screening must be up to date with current facility.
