Facility Evaluation Report

Licensing Program Analyst (LPA) Contreras arrived to conduct a Case Management visit and met with license Leonila Bunyi and Program Manager Orland Ordenez, to go over a recent Stipulation and Waiver; and Order that was issued on November 19, 2025. LPA went over the Stipulation and licensee, understands the conditions of the stipulation and fully understands that Staff (S1) is on Probation for a period of two (2) years from the Order dated of November 19, 2025. In addition, S1 shall not hold a position of Administrator in any licensed facility; S1 shall only provide care and supervision to clients in care at a facility only under the supervision of the licensee or another staff member.

S1 is to complete training as requested by the Department. S1 has fulfilled this requirement and submitted documentation to the Department.

Licensee Leonila Bunyi clearly understands the Stipulation in Order and violations of any terms of the Stipulation shall constitute sufficient grounds for the immediate exclusion of S1.

No deficiencies cited during today's visit.
