Facility Evaluation Report

At approximately 12:40 PM, Licensing Program Analyst (LPA) Contreras and LPM Mota arrived unannounced to conduct a Case Management - Incident Visit, and met with Administrator , Cynthia Dinglasan. The purpose of the visit was to obtain further information related to an incident report that was submitted to Community Care Licensing (CCL).

Incident Report 1: CCL received an incident report on 04/14/2025. Report states that on 4/14/25 at approximately 7:26AM, facility received two video clips sent by client C1 parent. Both videos show discoloration on the back of C1. C1 parent observed the discoloration today and felt that it happened to C1 at day program on Friday, April 11.

Administrator showed video to LPA and LPM, which appeared to be a rash and not bruising. Administrator stated C1 parent later confirmed that it was in fact a rash from possible hair dye and disappeared shortly after C1 was bathed. Parent apologized to administrator.
