The inspector’s account

At approximately 9:35 LPA Contreras and LPM Mota arrived unannounced to conduct a required Annual inspection and was greeted by Administrator Cynthia Dinglasan. Facility contact information was reviewed.The facility currently provides care for 45 clients.

At approximately 10:00am LPA and Admin toured the building and grounds. The facility was found to be clean and at a comfortable temperature. Water measured at 108.3 and 116 degrees F which is within the allowable range of 105 to 120 degrees F.

4 out of 4 fire extinguishers within the facility were found to be last charged 1/24/2025. Smoke and carbon monoxide detectors were last inspected in December 2024.Emergency disaster drills are conducted on a quarterly basis and documented.

LPA' conducted an inspection of 6 out of 6 facility vans and found all vans to be properly equipped with fire extinguisher and 1st aid kits. Van 4 was lacking a complete first aid kit and had a second kit which appeared to be dirty due to long term storage. LPA had discussion with administrator to ensure all items inside emergency kit are clean and sanitary. Administrator agreed to ensure all kits are updated and clean and replaced the kit during the visit. Fire extinguishers in the vans were found to be fully charged.

Toxins, cleaning supplies and other potentially dangerous items were observed stored in a designated cleaning supply closets inaccessible to clients. All cabinets and drawers had a magnetic lock inaccessible to clients.
