Facility Evaluation Report
Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a Case Management- Incident visit and was greeted by Program Manager Orland Ordanez. The purpose of the visit was to follow up on a self-reported incident that was submitted to Community Care Licensing (CCL).
CCL received an incident report on 5/01/2026 stating that on 4/30/2026 at approximately 7:36PM, the mother of a participant (P1) sent a video message in which the video showed skin discoloration and a scratch on (P1) right shin. Incident report stated the matter was reviewed the following day on 5/01/2026. Program Manager stated that participant was changed twice and no reported discoloration from two DSP that did the changing that day was reported. There was no signs that the bruising occurred during day program. In addition, facility driver reported no unusual incidents during transport. No further investigation was done from facility. Program Manager let mother know that no reported bruising occurred at facility. No further comments from mother was reported.
No deficiencies cited during today's visit. Copy of report given and read with program manager.
