Facility Evaluation Report
At approximately 9:45 AM, Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a Required - 1 Year inspection and met with Troy Richard, Licensee/Director. Facility provides in-person services, Monday through Friday from 9:30 AM - 1:30 PM. The facility is vendored by North Bay Regional Center (NBRC) and all the clients have a service coordinator. Transportation services are coordinated by the facility. Facility vans are inspected by a third-party vendor.
At approximately 10:00 AM, LPA initiated a tour of the facility with Director and observed the following: Facility is a one (1) story, was a comfortable temperature, and passageways were free from obstructions. Facility's fire extinguishers were observed charged and last serviced 6/25. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperatures in client accessible bathrooms read at 109.4 degrees F which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility maintains a supply of of non-perishable foods for emergencies, as well as an emergency water supply. Clients are responsible for bringing their own lunches. Facility has an internet access devise and internet available to clients in care, and the phone was tested and operational during today's inspection. There is a shaded seating area outside with outdoor space for activities. LPA was informed that facility does not store client cash or medications.
LPA observed clients engaged in activities with staff while other clients were able to choose their own solitary activities. LPA observed numerous cabinets containing activities such as crafting, art, movies, board games, exercising, dancing etc,. Director stated that clients participate in community outings such as bowling, restaurants, fire station visits, libraries, amusement parks, greater vallejo recreation department, volunteering at foods banks, volunteering with Meals on Wheels, etc,. Clients left on an outing during this visit.
