Facility Evaluation Report
At approximately 2:15, Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a health and safety visit for facility. LPA met with lead staff Joanna Hall. Facility currently has three clients in care. Facility is in the process of closing for a change of ownership.
Facility was found to be in good repair and at a comfortable temperature. Facility had all required postings. Cleaning products, sharp items, other toxins and chemicals are locked and inaccessible to clients.Extra linens and towels are kept in storage closet. Client rooms were found to be in good repair. LPA observed strong odor coming from one resident room. LPA observed all utilities to be operational including water, gas and electricity. Water measured at 126 degrees F not found within regulation of 105-120 degrees. LPA told admin to lower water heater and called staff to fix. Smoke and fire alarms were tested and operational. Fire exits were clear from obstruction. Fire extinguisher last inspected on 6/13/2025.
LPA observed a 7 day food supply of perishable and non perishable foods with expiration dates noted. Snacks were available to clients.
P&I monies were documented, secure and not commingled. P&I monies only handled for 1 of 3 clients.
Medication spot check was conducted and observed all medication to be centrally stored and secure
There were no deficiencies cited during today's visit,
