Facility Evaluation Report

At approximately 8:45AM, Licensing Program Analyst (LPA) Stefanie Mutialu conducted an unannounced Annual Required inspection to this facility and met with Singa Danle , DSP. At approximately 8:50 AM, LPA and DSP toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. The amount of fresh and non-perishable foods was within regulation. Toxins are secure and not accessible to clients. Medication is centrally stored and secure. There is a sufficient supply of hygiene products and linens on hand for client use. Two out three clients rooms were inspected and found to be within regulation. Each of the clients room is decorated according to clients preference and personality. Mattress pads were in place or available for Client use. Water temperature measured within regulation between 111 and 117 degrees F at three of three faucets accessible to clients. One of one combo Smoke/carbon monoxide detector, six out of six smoke detectors, and one of one carbon monoxide detector were tested and found to be in working order. Two out of two fire extinguishers were fully charged.

At approximately 10:10AM, LPA reviewed three of three Client records and two of three staff records, which were all found to be well organized. First aid and CPR certification were current in staff files reviewed. P&I monies were documented, secure and not commingled. Administrator's Certificate is expired as of 02/24/2024. DSP provided proof of Administrator Recertification Application, Education, and Cashed Check submitted on 01/06/2024 to LPA
