Facility Evaluation Report
At approximately 9:45 Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a required annual inspection and was greeted by staff Nancy Bermudez. Administrator (admin) Cecilia Morales arrived shortly after. Facility is an Adult Residential approved for 4 ambulatory clients. Facility has 5 rooms total with 2 rooms in use for clients and 3 for staff. Facility currently has 3 clients and all were attending day program during visit.
LPA toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed inside and outside facility. All required postings were in a highly visible area. Clients have access to outdoor use with outdoor exits and walkways. One bedroom is being shared while second bedroom is currently in use for one client. All bedrooms were observed to have clean linens and furniture.
LPA observed at least a 2-day supply of perishable and 7-day supply of non-perishable food. Facility kitchen, refrigerators and freezers were clean, and food was stored properly with expiration dates. LPA had conversation with admin that staff refrigerator must also have expiration dates. Toxins are stored in a locked closet and inaccessible to clients. Sharps and knives were locked in kitchen drawers. Alcohol was observed to not be locked under kitchen sink. Admin immediately removed and stored alcohol in a locked cabine t (Technical Violation given). All bathrooms have required bath mat and paper towels. Client's individual towels are hung up in restroom with residents name. Water temperature measured within regulation at faucets accessible to clients. Fire extinguishers were charged and inspected on 11/21/2024. Carbon and fire alarms were tested and operational. Disaster Drills are recorded and last drill conducted 9/08/2025. First aid kit available.
