Facility Evaluation Report
Licensing Program Analyst Contreras arrived unannounced to conduct a case management visit due to the change of administrator. LPA was greeted by Anirudh Wadwa who is currently the acting administrator for the facility and applicant. Current administrator certificate expired in 2025. Applicant is requesting an exception to be designated administrator although they do not meet all requirements.
LPA conducted COMP III Orientation was read with applicant who understood the regulations of an RCFE. Although applicant does not meet all requirements, applicant has been involved in day to day activities of facilities since 2016. Applicant was knowledgeable of current regulations and navigation of CCL website and resources available for licensees.
No deficiencies given during today's visit.
