Facility Evaluation Report
Licensing Program Analysts (LPAs) Jill Nakagawa and Ethel Contreras arrived unannounced on 01/29/2026 and met with Maria Villa, Supervisor, for the purpose of conducting a Non-Compliance inspection for the following compliance issues:
There were 20 residents at the time of inspection.
LPAs Nakagawa and Contreras met with Maria Villa, and toured the facility. Administrator Anirudh Wadhwa was off site at the time of visit but available via phone.
LPAs reviewed the above-mentioned compliance issues and found progress is being made.
A review in of the Plans of Corrections (POCs) found that the new Administrator has been working with CCL to clear POCs and pay fines. New Administrator is working with the Technical Support Program (TSP) to bring the facility into compliance.
