Facility Evaluation Report

Licensing Program Analyst (LPA) Nakagawa arrived unannounced for the purpose of conducting an inspection of the facility and to discuss staff training and maintaining staff training records. LPA met with the acting Administrator Anirudh Wadhwa.

LPA toured the facility with acting Administrator and found more renovations had been completed since visit of 11/25/2025.

LPA reviewed training records with acting Administrator and found that all staff had documented First Aid/CPR training, but complete training records for other requirements were only available for three of the six care staff. (Deficiency cited on 809-D). Technical Support Program (TSP) was discussed.

Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency, on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
