Facility Evaluation Report
Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced on 11/25/2025 and met with acting administrator to conduct a Non-Compliance inspection for the following compliance issues:
There were 18 residents at the time of inspection.
LPA Nakagawa met with the acting administrator Anirudh Wadhwa and toured the facility. LPA found that the Licensee had not yet resolved all of the above POC's and facility remains out of compliance, however progress has been made.
A Control of Property and Lease Agreement was provided to CCL The Licensee has assigned a new acting administrator to help in the day-to-day operations to help resolve compliance issues until new licensee has been approved and will take over all operations.
