Facility Evaluation Report
Licensing Program Analyst (LPA) Nakagawa arrived unannounced for the purpose of conducting an inspection of the facility and to discuss the following: construction at the facility, ongoing training, cleanliness, fire inspections, reporting of incidents and medication records. LPA toured the facility with staff member. LPA met with the acting Administrator Anirudh Wadhwa, who arrived shortly.
During the inspection, LPA found that the front of the building was taped off and renovations were being done. Administrator stated that new flooring and painting were being done throughout the facility, a section at a time in order to impact the residents as little as possible. Administrator stated that the Fire Department was aware.
LPA also reviewed training records with Administrator, who agreed to provide complete records within 10 days.
LPA reviewed the Centrally Stored Medication Records (CRMS) and the Medication Administration Record (MAR) and found them to be complete.
LPA inspected the kitchen and found an ample supply of perishable and nonperishable food as required per Title 22.
Exit interview conducted with Acting Administrator. Copy of report issued.
