Facility Evaluation Report

Licensing Program Analyst (LPA) Nakagawa, Licensing Program Manager, Kimberley Mota and Bethany Moellers, acting Regional Manager held an announced meeting to extend non-compliance supervision due to substantiated complaint investigations and case management inspections. Discussions were held with Sukhjit (Mantu) Sandhu.

On 10/22/2025 the Department held a Non-Compliance meeting to extend non-compliance supervision due to concerns at the facility. In addition complaint findings for Complaint # 21-AS-20251007163433. The facility was first placed on a non-compliance plan on 8/13/2021 and the plan has been extended on 10/9/2023 and 10/22/2025 due to continued non-compliance deficiencies. Current issues include:

* Administrator duties/qualifications (Current Administrator's certificate is expired 08/10/2025); newly assigned Administrator will file with DSS Regional Office in addition to the application made with CAB

The licensee has been cited for multiple violations of California Code of Regulations (CCR) and the Health and Safety Code (HSC). Licensee has failed to clear the Plans of Corrections by the dates required, use the Technical Support Program (TSP) program as recommended, and has continuously failed to operate the facility within Title 22 regulations.
