The inspector’s account

At approximately 10:00am, Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a Case Management Visit regarding recent change in administrator (admin). LPA was greeted by Anirudh Wadhwa whom will be the new administrator.

Administrator certificate is currently in pending status #7016455740. Application received on 8/18/2025.

Admin stated application was sent to CCLD regarding change of administrator but documentation was sent to Associate Governmental Program Analyst(AGPA) in Sacramento. Admin to forward information to LPA by tomorrow 8/28/2025 or may face potential citation and civil penalties from CCL.

LPA did a walk through with admin and observed rooms to be in fair condition. LPA discussed with admin moving resident to a different room due to poor conditions in current room resident is currently placed in. Admin agreed to have resident moved to different room.

The Department will review documentation for change of status.

No deficiencies cited during today’s inspection. Copy of Report given.
