Facility Evaluation Report
At approximately 10:00am, Licensing Program Analyst(LPA) Nakagawa and Contreras arrived unannounced to conduct plan of corrections visit and was greeted by Executive Director Eugenie Broussard.
LPA and Director did a walk through of the facility and observed all bathrooms to have paper towels, soap and trash cans. Deficiency was cleared from initial citation on 6/19/2025. LPAs observed bedridden resident was transferred to approved designated bedridden room. Deficiency was cleared from visit on 6/19/2025.
Director showed proof of admissions agreement for R1 and R2 from initial citation during visit on 6/19/2025.
No deficiencies given during today's visit.
