Facility Evaluation Report
LPA arrived at facility unannounced and no one answered the door. LPA attempted to call administrator(admin) Ricardo Menjivar but no answer received. LPA will attempt a second visit at a later time.
LPA was able to communicate with administrator but was unable to attend visit. Administrator gave verbal permission for Ana Colunga- Back up administrator to attend visit. Facility is an Adult Residential Facility licensed for a capacity of four (4) clients. Since being licensed, facility has not had any clients. Facility became vendorized February 2026 and are currently awaiting to receive their first client.
LPA and admin did a walk through of the inside and outside of the facility. Facility was found to be at a clean and comfortable temperature. LPA observed all walkways and exits to be unobstructed inside facility. All required postings were in a highly visible area. All bedrooms found in good repair. All bedrooms were observed to have required furnishing. Garage was observed to have tables and couch for client use. Due to no clients at facility, there is no food but refrigerator and freezers were clean and operational. Facility has emergency food and water. Fire extinguishers were charged and inspected on 4/29/2025. Carbon and fire alarms were tested and operational. Water measured at regulation between 105-120 degrees F. Chemicals locked. LPA observed clean linens and towels.
Staff has not been officially hired. No file review conducted. No client file review conducted. No medication or cash resources review conducted. Licensee to email CCL once there is a client move in.
Updated copies of the following documents to be submitted within 30 days of visit to CCL to update facility file:
No deficiencies given during todays visit. Exit interview conducted and report read with administrator.
