Facility Evaluation Report

At approximately 10:30am Licensing Program Analyst (LPA) Contreras arrived at a scheduled pre-licensing visit and met with administrator (admin) Ricardo Menjivar. This pre-licensing inspection is being conducted for an initial licensing. Facility is a one story, three bedroom, two bath home with a fire clearance approved for capacity of 4 clients. Rooms approved for non-ambulatory/bedridden, approval of delayed egress with secured (locked) perimeter.

LPA toured the facility with admin of the indoor and outdoor portions of the facility. Facility was found to be clean and at a comfortable temperature with emergency exits free from obstruction. Facility contained all required postings. 3 out of 3 bedrooms all had required chair, chest drawer and beds were made with appropriate linens. LPA observed emergency food and water supply with 7 day non-perishable foods available.

Outdoor area has patio and chairs for client use. There is an outdoor shed used for extra supplies and storage. Garage has lounge area for clients to access. Cleaning products, sharp items, other toxins and chemicals are locked and inaccessible to clients. Extra linens and towels were kept in storage closet. Additional supply of hygiene products available to clients. Facility has planned activities and art supplies available for client use.

LPA observed Fire extinguisher charged and last inspected April 28,2025. Fire alarm and carbon monoxide alarms were tested and operational. First aid kit available.
