Facility Evaluation Report
On 10/31/2025 Licensing Program Analyst (LPA) Ali Deniz conducted an unannounced Annual Required – 1 Year inspection visit at this facility and met with staff member Adeyi Foster . LPA contacted Licensee Zina Lee via phone call. Ms. Lee stated that due to the recent loss of a close family member, she would not be present for the inspection.
During the facility walk through with the staff member, she received another phone call indicating that an additional family member had passed away. Due to the staff member’s emotional condition, LPA decided not to complete the inspection at this time.
LPA will return at a later date to complete the annual inspection.
No citations were issued during today’s visit.
