Facility Evaluation Report

Licensing Program Analysts (LPAs) Lopez and Leibert conducted an unannounced case management inspection and met with Administrator, Noma Malik and Kayla Young, Assistant Executive Director. The purpose of these case management inspections were to follow up on current construction in facility. A few potential hazards were noted by LPA Lopez and Assistant Executive Director took action. LPAs also followed on some Administrator questions regarding residents.

No deficiencies cited during this inspection.
