Because residents in care are able to egress out of seven of the nine doors in the memory care the environment is not a locked, secured perimeter. Residents are not residing in a locked facility.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

No deficiencies cited. Exit interview conducted with Administrator, whose signature on form confirms receipt.
