Facility Evaluation Report

Licensing Program Analysts (LPAs) Stevenson and Cuadra arrived at the home for the purpose of conducting a closure inspection pursuant to voluntary closure of this licensed Adult Residential Care Facility (ARF). LPAs arrived and were allowed in by the Licensee Maria Golitzen. During previous visits made to the facility LPA observed learned that there were no clients in care.

During today’s visit, LPAs toured the facility, inspected all rooms, the exterior of the building and found no evidence that would suggest that any clients are residing on the premises including any clothing or personal items belonging to clients.

The Licensee initiated this facility closure with plans for voluntary closure and did submit a closure notice on 05/14/25 due to no placement of clients were assigned to the facility. On 05/14/2025 LPA received an email from Licensee notifying Community Care Licensing (CCL) about their plan to close the facility. Closure of this facility has been finalized. Facilityis closed effective 05/23/2025. Licensee turned over the License to LPAs.
