Facility Evaluation Report

On 03/16/2026 at approximately 10:20 AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct the 1-Year Required visit at this licensed Adult Residential Facility (ARF). LPA was greeted by staff member Lourdes Lapuz. The licensee, Rose Marie Devera, arrived shortly thereafter. The facility currently provides care for four (4) clients. At the time of the visit, three (3) clients were attending a day program. The facility has an approved fire clearance for four (4) ambulatory clients, with one (1) client permitted to be non-ambulatory. Upon arrival, LPA was informed that two (2) staff members were present at the facility. The facility is a single-story house with four (4) client bedrooms, two (2) bathrooms, one (1) staff room, and common living areas.

At approximately 11:15AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.

At approximately 11:45 AM, the LPA and Licensee conducted a tour of the building and grounds, which were observed to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted. LPA observed activity supplies for resident use. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Staff were in the process of cleaning up rooms at the time of this inspection. Toxins are stored in a locked housekeeping closet and under the kitchen sink. Water temperature measured 108.6 degrees F and 110.1 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to residents.
