Facility Evaluation Report
On 02/27/2026 at approximately 12:25 PM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct a Case Management visit at this licensed Adult Residential Facility (ARF). LPA was greeted by Administrator, Dinia Julian . Licensee, Rose Devera, arrived at approximately 12:30 AM.The facility has an approved fire clearance for a capacity of four (4) residents. Upon arrival, LPA was informed that there were four (4) residents in care and two (3) staff members present.
LPA conducted the facility’s last annual inspection on 04/18/2025. At that time, the facility had two (2) non-ambulatory residents and two (2) ambulatory residents. The Administrator indicated at that time that the facility wished to submit a new fire clearance request. LPA subsequently submitted a fire clearance inspection request to the Fairfield Fire Department. However, the licensee later communicated directly with the Fire Department Inspector and canceled the fire clearance request and indicated that resident's doctor marked the ambulatory status of one resident wrong, later facility corrected ambulatory status to ambulatory on resident's LIC602.
LPA conducted today’s Case Management visit to clarify the facility’s current ambulatory status, review resident documentation, and obtain copies of the facility’s current STD 850 and facility sketch. Copies of the requested documents were obtained during today’s visit.
