Facility Evaluation Report

During the course of a complaint investigation, Licensing Program Analyst Leibert, who arrived unannounced, noted that the three drinking fountains available to the clients are not functioning. This deficiency was noted during a prior visit conducted on 03/3/12/2024 and is being cited today.

The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal of rights provided.
