The inspector’s account

On December 19, 2025, Licensing Program Analyst (LPA) Ali Deniz arrived at the facility to conduct a closure inspection pursuant to the voluntary closure of this licensed Adult Residential Facility. LPA met with Administrator Wendi Counda. Human Resources Director, Kacy Goulart submitted written notice of the facility closure to the Santa Rosa Regional Office via email on September 22, 2025. The letter indicates that, following the Yolo Community Care Continuum’s meeting with Solano County on August 7, 2025, the Yolo Community Care Continuum determined to close the facility program.

LPA inspected all rooms and the exterior of the building and found no evidence indicating that any clients were residing on the premises. All client clothing and personal belongings had been removed. No personal household items were observed at the facility, as all items had been removed prior to or during the inspection.

Human Resources Director (HRD) Kacy Goulart initiated the facility closure and submitted a closure plan. On September 22, 2025, Licensing Program Analyst (LPA) received an email from the Licensee notifying Community Care Licensing (CCL) of the intent to close the facility. Per the Administrator, all clients were relocated to other licensed care facilities.

The closure of this facility has been finalized. The facility was closed effective December 19, 2025 . The Administrator surrendered a copy of the license during today’s inspection.
