The inspector’s account

On 09/25/2025 Licensing Program Analyst (LPA) Deniz conducted an unannounced case management Legal/ Non-compliance inspection to this facility and was greeted by Associate Director, Shemariah Lewis who contacted Administrator Wendi Counta and informed regarding todays visit. Administrator authorized staff to sign for today's visit. LPAs are following up on items that were concerning to ensure compliance at non-compliance conference dated 7/17/24:

Drugs brought and sold in the facility – Per Associate Director (AD) facility did not have any client who tested positive in last three months. AD stated drug tests and revisions of belongings are conducted periodically/randomly to monitor that this type of incidents are being prevented from happening. When the residents are back from outside visits, the facility conduct a drug test and check the client's belongings.

CCR 80064 (a)(2) Administrator - Qualifications and Duties - Administrator failed to ensure regulation requirements were met for facility operation. Based on the documentation reviewed and interviews conducted, the Administrator meets the qualifications outlined in Title 22, Section 85064 – Administrator Qualifications and Duties. Administrator holds a valid Adult Residential Facility (ARF) Administrator Certificate (#6013654735), which is active and set to expire on 09/29/2026.

CCR 80061 Reporting Requirements - Facility has failed to adhere to incident reporting requirements. LPAs reviewed incident report logs that confirmed that facility has been reporting incidents to CCL within regulations.
