Staff do not prevent altercations between residents
On 7/25/2024, Licensing Program Analyst (LPA) Tobola arrived unannounced for the purpose of delivering complaint investigation findings and was greeted by Clinician in Training, Dominique Coleman. Acting Administrator, Wendi Counta was notified and arrived later in the visit. LPA toured the facility, interviewed clients and staff and made observations during the course of the investigation.
Complaint alleges staff do not prevent altercations between residents. Upon interviews with several clients (C1-C5), information does not provide corroborating evidence towards the allegation. Clients (C1-C4) and staff (S1,S2,S3) indicated that they have not witnessed or heard of any physical altercations involving clients. C1-C4 also state that staff are proactive on de-escalating verbal altercations to prevent incidents from further becoming aggressive. Based upon review of records, LPA did not find any physical altercations reported within the past 6 months. Due to anonymous reporting party, a lack of contact and incident information provided and statements from clients (C1-C4) and staff (S1,S2,S3) indicating no witness or knowledge of client on client altercations, LPA is unable to review surveillance footage due to undetermined time frame of alleged incident(s).
Allegation, staff do not prevent altercations between residents is found to be UNSUBSTANTIATED. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
