COMPLAINT CONTROL NUMBER: 21-AS-20240614085849
On 7/25/2024, Licensing Program Analyst (LPA) Tobola arrived unannounced for the purpose of delivering complaint investigation findings and was greeted by Clinician in Training, Dominique Coleman. Acting Administrator, Wendi Counta was notified and arrived later in the visit. LPA toured the facility, interviewed clients and made observations during the course of the investigation.
Complaint alleges personal rights have been violated regarding visitation, internet access and phone contact and clients locked inside facility. Upon interviews with several client (C1-C5), information provided does not support the allegation and there is a lack of corroborating evidence to prove the alleged violation occurred. Clients (C1-C4) have indicated that there are no concerns with visitation, internet and phone access within the facility. In addition, clients (C1-C4) stated that clients are not locked in from the inside of the facility at any time of day.
Allegation, Personal Rights violated is found to be UNSUBSTANTIATED. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Appeal Rights given.
