Facility Evaluation Report
At approximately 1:20 PM, Licensing Program Analyst (LPA) Star Stevenson arrived unannounced to BEGIN a required 1-year annual inspection and was greeted by staff member Paul Baluot and Administrator Jenna Baluyot arrived at approximately 1:25 PM. Three clients were present during today's inspection and one (1) was away at the day program. Facility has a Hospice waiver for two (2). Facility is vendorized with North Bay Regional Center (NBRC).
At approximately 1:30 PM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one story home, was a comfortable temperature, clean and without odors, and passageways were free from obstructions. LPA observed the CCLD reporting poster and personal rights posters hanging in a back hallway to protect them from being repeatedly pulled down in times of client behaviors. Water temperatures in clients' bathrooms were slightly over the allowable range of 105 to 120 degrees F per Title 22 regulations and Administrator immediately lowered the water heater setting to ensure compliance.. LPA observed a supply of clean linens and hygiene, paper, and incontinent care products available for clients. Clients' bedrooms were inspected and 3 of 4 rooms were observed missing at least one (1) or more of the required furnishings as outlined in Title 22 regulations. Facility has care plans for these clients which reflect behavioral reasons for the removal of items, as well as, exception letters with licensing for the two clients. Facility has applied protection/bumpers at all the sharp corners of the facility to further protect the clients.
Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. In addition, facility has a large hard-wired back up generator and large railed ramps in the back and front of the property. The property has a large sun and shade patio and sitting areas. Two exterior sheds were unlocked to reveal additional medical supplies and various tools.
