The inspector’s account

On 02/18/2026, at approximately 11:10 AM, an informal meeting was conducted in the Santa Rosa Regional Office. Present in the meeting were Licensing Program Manager, Bethany Moellers, Licensing Program Analyst, Star Stevenson, licensee Robin Brandon PhD and administrator Raquel Cistac.

The purpose of the informal office meeting is to further discuss a self reported incident received by Community Care Licensing (CCL) on 2/7/2025. The Licensee was informed that this informal meeting is a part of the Administrative Action process and that further and/or repeat citations may result in a formal Non-Compliance Plan. A case management visit was conducted on 3/13/2025 and a subsequent visit on 6/19/2025, citation and civil penalty in the amount of $500 was issued for facility not seeking timely medical after observed change of condition of client (C1). C1’s change of condition resulted in hospitalization, C1 was diagnosed with acute abdominal pain from a large bowel obstruction. C1’s large bowel obstruction required surgical intervention to resolve and resulted in them having a subtotal colectomy. The civil penalty citation was cleared in a timely manner and facility continues to work to carefully observe clients and communicate amongst staff to ensure further timely medical care of clients.
