Licensing Program Analyst (LPA) Star Stevenson arrived unannounced to conduct a case management- incident inspection at approximately 9:40 AM. Staff and clients were not home upon my arrival and a call was placed to Regyna Jackson-Branchcomb who arrived at approximately 10:00 AM

LPA is conducting a case management visit to obtain more information regarding a self-reported Incident Report received by the Department on 12/08/2025, involving the injury of client (C1) and the resultant hospitalization on 12/06/2025.

No deficiencies were cited during todays visit.

Exit interview was conducted with Regyna Jackonson-Branchcomb whose signature on form confirms receipt of document.
