The inspector’s account

On 06/05/2025 Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr inspection for this facility and met with Dannell Merritt, Work Inclusion Manager. The facility currently provides care for 30 clients, 20 of which were in attendance during time of visit.

LPA conducted a tour of the facility with Work Inclusion Manager; facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Fire extinguishers were found to be last charged 01/2025. Facility has fire pull system alarm and was last inspected by the Fire Department 01/29/2025. Facilities last emergency disaster drill was conducted on 05/23/2025. Facility has three company vans. LPA observed one out of the two bathrooms to have an out of order sign, per conversation with Manager, the toilet is backed up and a plumber arrived during inspection to resolve the issue. Chemicals are stored in a designated cleaning supply closet inaccessible to clients. Facility has a first aid kit and emergency supplies. LPA observed clients in the behavioral support program engaging in activities, while other clients were on outings in the company vans. Clients have the option to pick and choose what they prefer for activities to participate in. LPA was informed clients bring their own lunches to program.

The facility does not handle either client cash resources or medications within the program. LPA conducted a file review of 5 client records. 1 out 5 clients did not have a physicians report on file, per conversation with manager they have requested it to be filled out by the physician and did not hear back, facility will reach out to physician again. In addition, LPA conducted a file review of 5 staff records and found staff to have the required amount of annual training and CPR & 1st Aid certification.
