Facility Evaluation Report
On 01/16/2026, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct a case management follow-up and met with the facility Program Director, Brittany Burnett.
Community Care Licensing (CCL) conducted a case management follow-up in response to a reported client death. The facility reported that while on an outing, a client choked on food at a restaurant. Facility staff and two bystanders attempted the Heimlich maneuver. Emergency medical services were contacted and arrived on scene; however, resuscitation efforts were unsuccessful, and the client was pronounced deceased.
LPA interviewed the Program Director and gathered additional information regarding the incident. The facility reported that a death notice would be submitted to the Department as required.
No deficiencies were cited as a result of this case management follow-up. Report provided to Program Director.
