The inspector’s account

At approximately 9:45 AM Licensing Program Analyst (LPA) Stevenson arrived announced to conduct an inspection in regards to the voluntary closure of this facility. LPA met with Licensee Margaret Mroz who initiated today's inspection closure by notifying Community Care Licensing (CCL) or her intention to close on 02/11/2026.

LPA toured the facility and observed no signs of clients residing at the facility. All client medications have been removed. All clients have been relocated to their new residences without incident. LPA was advised that last client moved out on 05/15/2026.

LPA will send out forfeiture letter to Licensee to complete the closure process.
