Facility Evaluation Report

An informal conference was conducted today in the Santa Rosa Regional Office. Present for the Santa Rosa Office were Licensing Program Manager Carla Martinez, Licensing Program Manager Bethany Moellers, and Licensing Program Analyst Star Stevenson.

Attending virtually for the Oakmont Management group were Scott Carlson-Senior Vice President (VP) of Operations, Tedra Godfrey-Regional Operation Specialist and Administrator at Ivy Park at Rockville, Terry Ervin VP-Quality/Clinical, Jimmy Duong-Regional Health Services Director, Sue McPherson-Senior VP of Regulatory, Jennifer Sato-Senior VP of Health Services and Safoora Ahmed-VP of Memory Care Programming.

The purpose of the informal office meeting was to discuss multiple complaints received by the Department. In 2025, there were eleven (11) complaints investigated and to date in 2026 there have been one (1) complaint investigation, and three (3) case management visits were the potential for financial or elder abuse were raised.
