Facility Evaluation Report

At approximately 9:45 AM Licensing Program Analyst (LPA) Star Stevenson arrived unannounced to CONTINUE a required 1-year annual inspection and was met by assistant administrator Angelica Gonzalodo who called Licensee/Administrator Selene Cruz-Astorga who arrived at 10:05 AM to assist with today's inspection.

At approximately 10:10 AM, LPA reviewed four (4) staff files and four (4) client files. Four (4) of four (4) staff files reviewed have all the required paperwork and proof of current First Aid and CPR training. Four (4) of four (4) client files reviewed have all the required paperwork. Staff coordinate medical and dental visits for the clients and take them to their appointments.

At approximately 10:45 AM LPA reviewed P&I monies and logs, which were organized, maintained, locked and stored in accordance with regulation. Facility had an accurate record of P&I monies, but needed to update their LIC400 Affidavit Regarding Client Cash Resources to reflect the higher amount on hand. In addition licensee called her insurance broker to increase her surety bond to cover the amount of cash resources on hand while this LPA was in the facility and a copy of the update Surety bond insurance was obtained for state file.

At approximately 11:15 AM, LPA reviewed medications and medication records which are maintained and stored in compliance with regulation.

Updated copies of LIC 500 Personnel Report, LIC610D Emergency Disaster Plan, LIC400 Affidavit Regarding Client Cash Resources and update an appropriate Surety bond was obtained today.

No deficiencies cited during today's inspection. Exit interview conducted with licensee whose signature on form confirms receipt of documents.
