The inspector’s account

On 02/23/2026 at approximately 09:30AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Adult Residential Facility (ARF). LPA was greeted by licensee, Carl Resurreccion and administrator, Aleha Devera. Facility has an approved fire clearance and capacity for 4 ambulatory clients. Upon arrival, LPA was informed that there were 4 clients in care and 3 staff members on-site. 3 of 4 clients were attending day program at the time of this visit. Facility is a 1 story building with 4 Resident bedrooms, 2 bathrooms, and 1 staff bedroom, and common spaces.

At approximately 10AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.

At approximately 10:35AM, LPA and Administrator toured the facility building and grounds, which were observed to be clean and in good repair. However, Bedroom #1 was found to be significantly disorganized, posing potential health and safety concerns. The client residing in this room has expressed that she does not want her room cleaned; however, at the time of inspection, there was a strong odor and limited space to safely ambulate within the room. The Administrator agreed to speak with the client regarding the concerns and ensure the room is cleaned. The Administrator will submit proof of correction, including photographs, to CCL by 03/03/2026 (Technical Advice Given). LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. LPA observed activity supplies for resident use. The supply of fresh food items was observed to be low at the time of the visit. Staff reported they were in the process of conducting their weekly grocery shopping during the inspection. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Toxins are stored in a locked housekeeping closet and under the kitchen sink.
