Facility Evaluation Report
On 10/03/2025 Licensing Program Analyst (LPA) Ali Deniz arrived at the home for the purpose of conducting a closure inspection pursuant to voluntary closure of this licensed Residential Care Facility for the Elderly. LPA met with Licensee, Emily Barbasal who submitted the written notice letter of facility closure to the Santa Rosa Regional Office via e-mail on 09/23/2025. The letter indicates facility will be closed due to difficulty getting clients from North Bay Regional Center. The facility vendorized by North Bay Regional Center and never received a client in last 14 months.
LPA inspected the building today and found no evidence that would suggest that any clients are residing on the premises.
The Administrator initiated this facility closure and did submit a closure plan. On 09/23/2025 LPA received a phone call from Licensee notifying CCL about their plan to close the facility. Per Licensee, no all residents, so no eviction letter was given.
Closure of this facility has been finalized. Facility was closed effective 10/03/2025.
Licensee turned over copy of License during today's inspection.
No deficiencies cited during today's visit.
