Facility Evaluation Report
On 06/30/2025 at approximately 11:15AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Adult Residential Facility (ARF). Upon arrival LPA didn’t get response from the entrance door and contacted to the administrator Emily Barsabal. She arrived 15 munities later. Fire Clearance has been approved for 4 ambulatory clients. Upon arrival, LPA was informed that there were no clients in care and no staff member on-site. Facility is a 1 story building with 4 Resident bedrooms, 2 bathrooms, and common spaces.
At approximately 11:50AM, LPA and Licensee initiated a tour of the facility and observed the following: Facility is a single-story home, was a comfortable temperature, and passageways were free from obstructions. Water temperature in clients' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens and paper products available to clients. Clients' bedrooms were inspected and observed to have appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk had lock.
Facility has at least two days of perishable and one week of non-perishable foods. There is outdoor space for activities. There is one shed in the back yard, which were observed to be used for storage and projects. LPA observed activity supplies for clients’ use. Licensee states that they are having hard time to find clients since they licensed. Licensee is currently in partnership with North Bay Regional Center and vendorized for client placement.
