The inspector’s account

On 03/11/2026 at approximately 9:30AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct a Required 1 Year visit and met with Program Director, Gabby Echevarria. Facility is an Adult Day Program for Adults with Disabilities. Facility has an approved fire clearance and capacity for 90 Clients, of which 10 Clients can be non-ambulatory. Upon arrival, LPA was informed that there were 37 clients attending Day Program and 16 staff members on-site. Facility is a one-story building with activity classrooms, staff offices, and common areas. Facility provides mostly in-person services, Monday through Friday from 8:30 AM - 2:30 PM. Facility is vendorized with North Bay Regional Center (NBRC).

At approximately 10:45AM, LPA and Program Director toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. LPA observed activity supplies for clients use. Chemicals and other items which could pose a risk to clients in care stored in locked cabinets and closets. Soap & paper towels were available in facility bathrooms. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Clients bring their own lunch. Clients doesn’t have access to the kitchen. Water temperature measured 106.6 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to residents.

Fire extinguishers were charged and were last serviced 03/2025. Administrator stated that inspection scheduled for fire extinguishers on 03/12/2025. Carbon monoxide detector located in main common area was tested and found to be in working order. Facility doesn’t manage client’s medication and P&I money.
