Facility Evaluation Report
At approximately 1:30 PM , Licensing Program Analyst (LPA) Stevenson arrived to BEGIN a required – 1 yr. inspection visit for this facility and was greeted by licensee/administrator Dante Ward. Facility is a single story home and licensed for four (4) ambulatory clients. Facility is vendorized with the local Regional Center. Facility has an infection control plan on file as required. There are currently 4 clients in care. Upon arrival all clients were away at day programming and at approximately 1:50 PM two (clients) arrived back home.
At approximately 1:40 PM LPA and licensee toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Refrigerated food was found to be stored and labeled in a safe manner.
All rooms were equipped with lighting, night stand, and chest of drawers.
Extra hygiene products and linens were available. Water temperature in sinks accessible to clients in care were measured to be within the range of 105 to 120 degrees F. Fire extinguishers were last inspected 02/2025 and found to be fully charged. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Toxins, sharps and other items that could pose threat if available to clients we found to be secured.
Medicines were observed to be centrally stored and locked.
Licensee was advised that LPA will return at a later unannounced date to complete facility file inspection, medicine administration and P&I money accounting.
