Facility Evaluation Report

At approximately 07:25 AM Licensing Program Analyst (LPA) Stevenson conducted an unannounced case management inspection regarding an Immediate Exclusion Order for now former staff member (S1). S1 was banned from working at any facility licensed by Community Care Licensing (CCL) effective 02/02/2026.

LPA met with caregiver Aira Abeleda who has Designation of Facility Responsibility (RP) who confirmed the previous employment termination of S1. RP called Griffin Family Care Home manager, who also confirmed the termination of S1.

S1 is now noted to be dissociated from the facilities Guardian roster and based on evidence obtained during today’s inspection, the LPA has verified the individual is not present, employed, or residing at the facility.

LPA is requesting the following for the facility file.

Termination letter for S1 indicating their last day of employment.
